In the afternoon of Aug.16.2019, “the First Study Class of Internal Fixation for Periarticular Fracture in The Sixth Hospital in Shanghai” was opened in the meeting room in 1^st floor of Orthopedic building in The Sixth Hospital in Shanghai. More than 50 orthopedic surgeons from hospitals across the country participated. The opening ceremony of the study class was hosted by Professor Chai Yimin who also made the welcome speech. At the meantime, he welcomed and thanked for the guest lecturer- Professor Robert Medoff, director of orthopedics, royal Hawaii medical center.

For the first course, Professor Robert Medoff reviewed some knowledge of imaging of distal radius fractures and shared some valuable clinical cases files that he encountered during his many years of clinical work, illustrating the importance of body position and X-ray film reading techniques in guiding clinical treatment.

In the second session, Professor Wang Qiugen talked about the treatment of the edge shear fractures in distal radius and comminuted fractures, as well as some difficulties that still existed in clinical practice. For example, for the treatment of ultra-distal comminuted fracture, due to the lack of tools and means, we had to choose some relatively lagging surgical methods, so it was difficult to achieve satisfactory results in treatment.

 With respect to the difficulties mentioned by Professor Wang Qiugen, Professor Robert Medoff put forward a perfect solution in TriMed’s fixation system established by him. This improved method had been included in “Campbell orthopedic surgery” and “Green hand surgery”, becoming the model of periarticular fracture treatment in the United States.

The second session of “The Study Class of Internal Fixation for Periarticular Fracture in The Sixth Hospital in Shanghai” began at 8:00 in the morning of Aug.17, 2019. The attending teachers arrived in advance. Professor Shi Zhongmin, foot and ankle study group member, orthopaedic branch of the Chinese medical association, and the host of this conference made the opening speech. He welcomed Professor Robert Medoff and announced the beginning of the meeting today.

 

Professor Robert Medoff reviewed the AO core concept with everyone.

 

During the first link of the conference, Professor Robert Medoff reviewed the AO theory basic knowledge for treating orthopedic fractures. We all knew the four principles of AO: anatomical reduction, strong fixation, protection of blood supply, early active exercise. Professor Robert Medoff believed that the concept of AO was very good for the treatment of long shaft bone fractures. For the fixation of these fractures, there were some important factors, including the fixation on the tension side and the use of screw holding force, etc. In the fixation of long shaft bone, the steel plate should have sufficient length and strength.

According to the AO manual, the use of a lag screw was a very important step. It was used to resist the shearing and twisting force of the bone fragments, and it should not be screwed too tightly, or the fixation would fail. As for comminuted fracture, or osteoporosis, how to fix it? A lag screw needed to be strong enough on the opposite side of the cortex, but there were some occasions that the opposite side of the bone was so small that there was no bone left, so there was not enough space for the lag screw. Similarly, we cannot use some large plates either. Therefore, Professor Robert Medoff found that the treatment concept of periarticular fractures was different from that of general fractures, so new fixation methods must be considered to eliminate the dependence on screw holding force. The feline, for example, had long canine teeth that allow them to grip their prey tightly, so he designed and invented HOOK plates. The sled plates were designed and invented based on the iron hoop on the outside of the whisky barrel, which was associated with the arch of Rome. The pin at the distal end of the radial column pin plate was small and had a very low height, but why was it powerfully supported? This was because using the suspension bridge principle. Your mind can't always be tied to the idea of AO.

Director Li Enchao from Jen Ching Memorial Hospital put forward a question: Should the TriMed sindwinder plate be placed on the periosteum or under the periosteum after dissection? Professor Robert Medoff answered: After a small amount of dissection, the steel plate should be attached, and the front and rear positions of the steel plate on the fibula should be confirmed under direct sight, and the fracture end can be reduced by clamping with reduction forceps, without too much periosteal dissection, so as to avoid affecting the fracture healing.

Professor Shi Zhongmin made the summary speech: What we all received the fracture treatment education was first reduction fracture, then temporary fixation, finally put on the plate, screw, and this set of fixed procedure. However, what we heard today was different from the original. TriMed internal fixation system used steel plate to reduce the fracture and then used screws to fix it. You should follow the new concept when using TriMed internal fixation system to treat periarticular fractures, so as to achieve a satisfactory reduction effect.

Finally, Professor Zeng Bingfang, a highly respected professor, gave a speech at the closing ceremony of the conference.
